Athletic Requirements and Eligibility
All required participation forms, including the KHSAA MS Sports Physical, must be uploaded to FinalForms. Once you create your account, you will only need to upload a new physical each year and make sure all of the basic information is still correct. All sections in FinalForms must be completed before participating in conditioning, tryouts, practice and competition.

Barret Traditional Middle School and the Jefferson County Public Schools believe that all student athletes should be successful in the classroom. Therefore, all students who participate in school-sponsored activities must remain academically eligible according to Barret and district guidelines.
